Patients Rights Notice
In accordance with the No Surprises Act, a federal law enacted to protect patients from unexpected medical bills, healthcare providers are required to give an estimate of the expected costs for patients who are uninsured, or not using insurance, for their care.
You are entitled to receive a Good Faith Estimate detailing the overall anticipated cost of any non-emergency services, including dental services.
Your provider must give you a written Good Faith Estimate at least one business day before your scheduled service or item. You can also request an estimate from your healthcare provider, or any other provider, before booking a service.
If the bill you receive is $400 or more than your Good Faith Estimate, you have the right to dispute the charges. Be sure to keep a copy or photo of your Good Faith Estimate for your records.

If you do not have insurance or choose not to use it, please inform your dental provider, and they will provide you with a Good Faith Estimate of your expected costs.
